Feel free to call or email for any questions or special arrangements: 512.732.9285 or hhubbell@eanesisd.net If your child has a life threatening allergy and needs an Epipen, please have your doctor fill out an Emergency Allergy Action Plan and return it to the nurses before school starts in the fall. The form can be accessed here: http://whshhubbell.weebly.com/forms.html WHS students need to carry their own Epipen. Stock Epipens are kept in the clinic. In the absence of a current student action plan, we will use the EISD standing policy. Any other major medical conditions such as diabetes or seizures need a doctor's plan submitted. The seizure plan and diabetic plan is located on the nurse web page. http://whshhubbell.weebly.com/ If your student needs to take prescription medication next year, please have your doctor fill out the medication permission form found here: http://whshhubbell.weebly.com/forms.html. In the fall, you may bring the medication to the clinic in the original bottle with a current prescription. OTC meds may be carried at WHS. You may read the medication policy on the nurse web page found here.http://whshhubbell.weebly.com/forms.html Please check with your doctor to make sure that your student does not need any immunizations over the summer. Shot waivers are good for 2 years and must be current at the start of school.
